Financial Aid Forms

To complete your financial aid application, we may have requested you submit additional forms.  Below are two lists of forms:

• one for the 2023-2024 academic year (for fall 2023, spring 2024, and summer 2024), and
• one for the 2024-2025 academic year (for fall 2024, spring 2025, and summer 2025).

Please contact us if you are unsure which forms you are required to submit.

Include your UW-W student ID on all forms and documents. Ensure that documents have the required signatures. Forms can be faxed, mailed or dropped off in person to the Financial Aid Office. Forms may also be scanned and emailed to the Financial Aid Office at uwwfao@uww.edu. We do not recommend emailing any documents which contain personal information (i.e., social security number, date of birth, driver’s license number, banking information, etc.).
