Criminal Background Check (CBC)

You have now enrolled in the Edfoundprc 210 (and likely other courses included in the Foundations Block). 

1. Check your schedule and make sure the ONLY classes you are enrolled in on a Tuesday or Thursday are: Edfoundprc 210, Edfound 222 or 212, and Edfound 243.  If any of your other classes are held on a Tuesday/Thursday, you must change that now.

2. In order to remain enrolled in Edfoundprc 210, you must complete ONE of the options below.  Please, read the instructions carefully so you fill out the correct forms based on the section of the course you enrolled in.

The University of Wisconsin-Whitewater College of Education and Professional Studies requires a criminal background check to be completed for all students working in the field.  The College of Education and Professional Studies will receive your results and distribute them to your field based site.  If there is history present on your background check, you will be notified by the Office of Field Experiences via email with further information.  If you are not contacted, no further action on your part is required.


In order to be placed in a Milwaukee Public School (MPS) for eight Tuesdays, you must complete the background check form linked below, sign it, and return it to Winther 2003 (or email it to schwarza@uww.edu). Any inaccurate or missing information could result in a denial by MPS. You must complete this form in order to remain enrolled in the foundations block courses. If the form is not completed within a week of enrollment in the course, you will be removed from the course. We will send the completed background check form to MPS for processing prior to the beginning of the semester. Therefore, if you drop the class OR change sections, you must notify Alice Schwarz (schwarza@uww.edu) immediately.  If your form has already been processed, you will be responsible for the $12 fee.

Milwaukee Public Schools Criminal Background Check

There are TWO forms needing to be completed in order to remain enrolled in the hybrid section of Edfoundprc 210. These forms are linked below. Please complete both forms, sign and date them, and bring them to Winther 2003 or email them to schwarza@uww.eduYou must complete the forms in order to remain enrolled in the foundations block courses. If the forms are not completed within a week of enrollment in the course, you will be removed from the course.

Hybrid Section Criminal Background Check

In order to be placed in a Beloit school for eight Tuesdays, you must complete the forms linked below and deliver them to Winther 2003 or email them to schwarza@uww.edu. We will send the completed background check form to Beloit for processing prior to the beginning of the semester. Therefore, if you drop the class OR change sections, you must notify Alice Schwarz (schwarza@uww.edu) immediately. If your form has already been processed, you will be responsible for any fees incurred. You must complete this form in order to remain enrolled in the foundations block courses. If the form is not completed within a week of enrollment in the course, you will be removed from the course.
