UHCS Wellness Program Request Form
The UHCS Wellness team works with our Wellness Peer Educators and physical and mental healthcare providers to provide free programs that are available to your staff, class, hall, floor or organization. You can choose from the list or request a custom program by clicking the link above. Please complete the form at least two weeks in advance of your preferred date. After requesting a program, wellness staff will follow up with you to confirm. If you have questions, please contact wellness@uww.edu.
